2. Vine Congregational UCC Foundation Grant Application (External) – complete this form if you are submitting a request on behalf of an organization and/or person in the wider community of Lincoln/Lancaster County. 

 

3. Donor Directive form – complete this form to direct funds to the Vine Foundation. Your completed Donor Directive form will be kept by the Vine Church Financial Secretary so that your gift will be directed according to your wishes.
